KSYC 2024 WAIVER: A waiver is required for all minor participants attending KSYC 2024, meaning those who will be under the age of 18 years old at the beginning of the convention, November 22, 2024. Please enter participant information below, then fill out appropriate names and signatures. A parent/legal guardian signature is required for all participants under the age of 18. Parents/guardians may sign for up to three minor children on the same form, so long as the names and birth dates of the children are included. SIGNING INDICATES THAT YOU HAVE READ, UNDERSTOOD, AND AGREE TO THE WAIVER.
